Take a look at GNU Mach[1][2][3].  (You need the unreleased GNU Mach 
2.x, not 1.3.)  Start from oskit/x86/main.c.  Although this may not be 
your case, it can help you.  The downside is that GNU Mach is not a 
simple kernel.
